A weekly communication form is required to be submitted in the field practicum classroom through canvas, starting week one though week 16. This form includes an overview of hours performed and weekly learning opportunities linked to the learning plan and competencies. It is encouraged for students to share this weekly communication form with their field instructor as a helpful tool during supervision, however it is not required to share the complete form. Students must have their Field Instructor or in approved cases their task instructor sign on off on weekly hours. This form will also serve as a running time log for the number of hours completed within the field placement for the semester as well as the total number of hours for the entire practicum. The communication form is also an opportunity to voice any potential concerns about their agency or field instructor. This classroom instructor reads and comments on the communication form each week through Canvas. The form should be treated as part of student practitioner responsibilities for your agency practicum. The form is graded based on full completion, timeliness, task, activity, and agency reflections that are related to social work competencies.
